Here are three easy steps to make an insurance claim

Step 1

Complete Claim Notification Form

You may download the Claim Notification Form here.

Step 2

Attach additional relevant documents

Medical reports completed by the treating doctor such as diagnosis note, proof for duration of hospitalization, Driver license (for motor vehicle accident only), post mortem report/ autopsy report and death certificate for accidental death.

Police report and investigation findings on the alleged accident or incident report by company for industrial accident.

Letter of Administration/ Distribution Order (if no nomination) and a copy of Identity Card with supporting relationship of the person(s) receiving the policy benefits payment for death.

Medical Specialist Report confirming the Permanent and Percentage Disablement at the end of specified period.

Step 3

Submit to AIG within 30 days of incident

Mail the completed Claim Notification Form and all the required supporting documents to the following address:
